Please complete the Flu Appointment Request form on the patient portal to schedule your child's appointment. Parents can also get theirs at the same time! Cost is $30 for parents.

10/21/24 (Monday): 9:00am-12:45pm, 2:30pm-4:30pm
10/23/24 (Wednesday): 3:00pm-5:45pm
10/25/24 (Friday): 2:30pm-4:45pm
11/5/24 (Tuesday): 8:30-12:30pm
11/6/24 (Wednesday): 2:30pm-5:30pm
11/12/24 (Tuesday): 4:00-6:00pm
11/19/24 (Tuesday): 3:00pm-6:00pm
