What is My Kid’s Chart?
My Kid’s Chart is an online patient portal that patients and families can access 24/7 to:
- Generate your child’s immunization record
- Review visit histories
- Check the date and time of next scheduled appointment
- Request prescription refills
- Send questions, school/camp forms or other requests through secure messaging.
What is Secure Messaging?
The secure messaging feature allows you the ability to send non-urgent messages to the office and doctors at any time. Please be aware that the messages will be checked during normal business hours, Monday-Friday, 8am-5pm. If you have an immediate need, please call our office. As always, if you have a medical emergency, please call 911.
In addition, files such as pictures and forms may be uploaded as well. For school and daycare forms, we will complete the form and send back to you via message. Please allow a turnaround time of 48-72 hours for the completion of the forms.
How Do I Sign Up?
To sign up for My Kid’s Chart, please complete this formand fax it to us at 281-485-9469 or complete a form next time you are in the office. You will receive an email within 2 business days with your username and temporary password.
